Terms and Conditions of Entry

  1. The Promoter
    1. The promoter is: CFH Docmail Ltd. of St. Peter’s Park, Wells Road, Westfield, Radstock BA3 3UP.

  2. The competition
    1. The title of the prize draw is the CFH Docmail Health Research Survey Prize Draw 2020.
    2. Each GP practice may make one entry into our free prize draw.

  3. How to enter
    1. You can enter between 9am on Friday, 3rd July 2020 (the "Opening Date") to 5pm on Friday, 10th July 2020 (the "Closing Date") inclusive. The Promoter reserves the right to extend the Closing Date at its sole discretion.
    2. All prize draw entries must be received by the Promoter by no later than 5pm on the Closing Date. All entries received after the Closing Date will be disqualified.
    3. To enter the prize draw, simply complete the attached Docmail survey and enter your contact details where indicated and we will automatically include you in the prize draw.
    4. No purchase is necessary and there is no charge to enter.
    5. The Promoter will not accept responsibility for entries that are lost, mislaid, damaged or delayed in transit, regardless of cause, including, for example, as a result of any postal failure, equipment failure, technical malfunction, systems, satellite, network, server, computer hardware or software failure of any kind.
    6. By submitting a prize draw entry, you are agreeing to be bound by these terms and conditions.

  4. Eligibility
    1. By submitting a prize draw entry, you are agreeing to be bound by these terms and conditions. For the avoidance of doubt, the following are excluded from entry:

      a) employees of the Promoter or its holding or subsidiary companies;

      b) employees of agents or suppliers of the Promoter or its holding or subsidiary companies, who are professionally connected with the competition or its administration; or

      c) members of the immediate families or households of (a) and (b) above.

    2. The Promoter may require entrants to provide proof that they are eligible to enter the prize draw.
    3. There is a limit of one entry to the prize draw per GP practice.

  5. The prize
    1. The winner will receive £150 towards the purchase of pizza, (drinks and sides are included) from a restaurant or takeaway of its choosing. There will be no prizes for other entrants.
    2. There is no cash alternative for the prize.
    3. The Promoter reserves the right to substitute the prize with a prize of equal or greater value.
    4. The prize is not negotiable or transferable.

  6. Winners
    1. The winner will be drawn at random and no correspondence or discussion will be entered into.
    2. The Promoter will contact the winner as soon as practicable after the Closing Date, using the telephone number or email address provided with the prize draw entry.
    3. The Promoter must either publish or make available information that indicates that a valid award took place. To comply with this obligation the Promoter will publish the surname and county of the winner on our website at www.cfh.com.

  7. Claiming the prize
    1. The winner can claim their £150 prize by purchasing their chosen pizza (drinks and sides are included) themselves and sending a copy of their receipt to the Promoter at the above address. The Promoter will then reimburse the winner as soon as possible on receipt.
    2. If the winner cannot be contacted or is not available, or has not claimed their prize within 30 days of the Closing Date, the Promoter reserves the right to re-draw the winner from those entries that were received before the Closing Date.

  8. Limitation of liability
    1. Insofar as is permitted by law, the Promoter will not in any circumstances be responsible or liable to compensate the winner or accept any liability for any loss, damage, personal injury or death occurring as a result of taking up the prize except where it is caused by the negligence of the Promoter or that of their employees. Your statutory rights are not affected.

  9. Ownership of prize draw entries and intellectual property rights
    1. All prize draw entries and the surveys submitted to the Promoter will become the property of the Promoter on receipt and will not be returned.
    2. By submitting your survey responses, you agree to:

      a) assign to the Promoter all your intellectual property rights with full title guarantee; and

      b) waive all moral rights,

      in and to the survey responses to which you may now or at any time in the future be entitled under the Copyright, Designs and Patents Act 1988 as amended from time to time.
    3. You agree that if you are the winner the Promoter may make your GP practice name available on its website www.cfh.com and any other media, whether now known or invented in the future, and in connection with any publicity of the prize draw. You agree to grant the Promoter a non-exclusive, worldwide, irrevocable licence, for the full period of any intellectual property rights to use, display and publish, your GP Practice name for such purposes.

  10. Data protection and publicity
    1. The Promoter will only process any personal information as set out in Privacy Policy.

  11. General
    1. If there is any reason to believe that there has been a breach of these terms and conditions, the Promoter may, at its sole discretion, reserve the right to exclude you from participating in the prize draw.
    2. The Promoter reserves the right to hold void, suspend, cancel, or amend the prize draw where it becomes necessary to do so.
    3. These terms and conditions shall be governed by English law, and the parties submit to the exclusive jurisdiction of the courts of England and Wales.
